Health Plans, Inc., (HPI) along with our authorized business partners, provides all information posted on this website and the mobile applications (collectively referred to as “Online Platforms”) we make available to our health plan members. This privacy policy describes the types of information we collect through the Online Platforms, how we use such information and to whom and under what circumstances we may disclose it. While we try to maintain only current and accurate information, we disclaim any implied warranty or representation about its accuracy or completeness, or appropriateness for a particular purpose. As the recipient of this information, you assume full responsibility for using the information available on the Online Platforms, and you understand and agree that HPI is neither responsible nor liable for any claim, loss or damage resulting from its use.

The mention of specific products or services on the Online Platforms does not constitute or imply a recommendation or endorsement by HPI unless it is explicitly stated. The views and opinions of authors do not necessarily state or reflect those of HPI. The Online Platforms may provide specific health information, none of which is intended to be specific to your medical condition. Nothing contained, expressly or implied, on the Online Platforms is intended or shall be construed as medical or other professional or financial advice by HPI.

The Online Platforms may contain links to other websites for your convenience. However, HPI disclaims any responsibility for the content, information or the privacy and security policies contained on those other websites, and does not exert any editorial or other control over those sites.

When we link to the sites of HPI vendors where Protected Health Information (PHI) is collected on our behalf, those sites are required to safeguard information in accordance with HIPAA.

Linked sites where PHI is collected have privacy and security policies covered under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

HPI does not expressly or implicitly recommend or endorse the specific services or products referenced at other websites linked to HPI's Online Platforms unless explicitly stated. The services, products, or providers on the Online Platforms, linked websites, or in listings of network doctors and hospitals may not be covered in your particular benefit package or included in your provider network. We offer this information to be comprehensive, but it is not linked to your benefits plan. You must consult your specific benefits plan, provider network or contact Customer Service if you have any questions.  

Privacy:
Browsing our Online Platforms
In the normal course of visiting the HPI Online Platforms, viewing pages or downloading information, we will gather and store certain information about your visit automatically. We use this information to help us make the Online Platforms more useful to visitors – to learn about the number of visitors to our Online Platforms and the types of technology our visitors use.

We (or a third party on our behalf) may automatically collect and store the following types of information about your visit:

  • The pages that you visit and links that you interact with on the Online Platforms 
  • The domain from which you access the Internet (for example, aol.com) 
  • The Internet address of the website, if any, from which you came to our Online Platforms
  •  The date and time you arrived at our Online Platforms and how long you spent visiting the Online Platforms
  • The name and version of your computer’s operating system (for example, Windows 11) and internet browser 
  • The pages you visit 

Additionally, our Online Platform may collect or allow you to enter personal information to access the Online Platforms or when using certain features or services, such as when creating an account or submitting for reimbursement. We use this information to provide products and services related to your health plan and administer your account.

Cookies
Some pages on the Online Platforms employ browser session “cookies”. A cookie is a small piece of data that a website transfers to your computer’s hard disk to allow the server to distinguish your session from that of others while you are connected. Requests to send cookies are not designed to collect information about you, but only about your “browser session.” The cookies make it easier for you to use the dynamic features of our Online Platforms. The cookies and the information about your session are automatically destroyed shortly after you close your browser. The information is not permanently stored on your computer. We do not collect personal information about you except as described above. 

Use of Personal Information (PI) and Protected Health Information (PHI)
HPI is committed to protecting the privacy and confidentiality of our members’ PI and PHI. We collect and disclose such information only according to our corporate policies and procedures and federal and state laws, including the HIPAA Privacy Rule. The HIPAA Privacy Rule took effect on April 14, 2003, and requires plan sponsors to provide members with a Notice of Privacy Practices. Unless otherwise permitted by HIPAA, we will not disclose to third parties that are not our vendors any identifying data, including name, mailing address, email address, and phone number, unless you authorize such disclosure or such disclosure is required by law. To ensure privacy of your data, never share your username and password with anyone. Be sure to consider the content of any questions, the location of your email account and who, besides yourself, may have access to it.

HPI and our authorized business partners collect personally identifiable data through our Online Platforms in the following ways:

  • Through members' secure online accounts and mobile applications; and 
  • Within "Contact Us" — Lets users send a secure inquiry to HPI Customer Service 
  • via email sent directly to HPI 

We will use your PHI/PI to operate the Online Platforms and to assist with administering the health plan and other applicable services that have been made available to you.

Online Platform security
Security is a priority at HPI. Appropriate measures are taken to protect the confidentiality, integrity and availability of information about our members, providers, employers, brokers and anyone else that may want to use our Online Platforms.

Only authorized users are allowed to access PHI. HPI enforces a high level of protection for securing transmissions between a user’s browser and the Online Platforms. Content on the Online Platforms is protected against unauthorized access.

Email security
Please be aware that communications sent using unencrypted email are not inherently secure. It is possible that they can be intercepted while in transit and read. HPI cannot guarantee the confidentiality or security of any information you send to us over the Internet when using email. HPI shall not be liable for any breach of confidentiality resulting from such use of email via the Internet.

HPI maintains a corporate email policy governing the use of email when communicating confidential and/or health information, which includes implementing reasonable and appropriate safeguards when communicating via email.  

Communicating with us
Medical questions or concerns about your care or treatment should be directed to appropriate health care professionals. HPI will not respond to requests for personal medical advice made through the Online Platforms. If you require an immediate response to non-medical questions or problems, please call Customer Service at 800-532-7575.

Email forms on our Online Platforms should not be used for confidential personal medical issues, but for general topics or plan questions. If you need to discuss personal information related to your benefits, you should call HPI Customer Service and not use email. Comments and questions submitted by email will be shared with our Customer Service Coordinators and/or those employees most capable of addressing your questions and concerns. Please be aware that your email may not be encrypted and may be accessed and viewed by other Internet users without your knowledge while in transit to us. See Email Security above.

Changes to this policy
HPI may update this policy from time to time, and will provide notice of any such changes to the policy as required by law. The date the privacy policy was last updated is identified at the top of the page. We will notify you of changes to this policy by updating the "last updated" date and posting the updated policy.
